Diffuse axonal injury

Diffuse axonal injury is widespread tearing of nerve fibers caused by the brain rotating violently inside the skull. It is one of the most severe brain injuries and often does not appear clearly on a standard CT scan.

What is diffuse axonal injury?

Diffuse axonal injury is damage to the long connecting fibers that carry signals between regions of the brain. Rotational force causes these fibers to stretch and tear across widespread areas.

The mechanism is different from a focal injury such as a bleed or contusion. Nothing is localized, which is what the word diffuse describes.

It occurs in high energy events, particularly vehicle collisions where the head rotates rapidly, and it is a common finding in severe trauma.

Damage also continues after the initial event, as injured fibers deteriorate over hours and days rather than failing immediately.

It sits at the severe end of traumatic brain injury and frequently accompanies other injuries in high energy collisions.

Why is it hard to detect?

The imaging picture frequently understates the injury.

The gap between what imaging shows and what the person experiences is the defining difficulty in these cases.

  • Standard CT often appears normal or shows only subtle findings
  • Damage is microscopic and spread across many regions
  • Specialized MRI sequences detect more than routine imaging
  • Advanced techniques such as diffusion tensor imaging can show fiber tract disruption
  • The visible findings frequently understate the functional impairment

What are the consequences?

Outcomes range widely. Severe cases produce prolonged unconsciousness and profound permanent impairment, while milder ones leave deficits in attention, processing speed and executive function.

Because the injury disrupts connections rather than destroying a single region, the effects are often described as things taking longer and being harder than before rather than as a specific lost ability.

That character makes the injury genuinely difficult to convey, since the person may appear unchanged in brief interaction.

How is it established in a claim?

Through appropriate imaging combined with clinical and functional evidence. Where routine CT is normal, specialized MRI sequences may be needed.

Neuropsychological testing documents the functional consequences, and testimony from family and colleagues establishes the change from baseline.

The mechanism of the crash matters as corroboration, since rotational forces consistent with the injury support the diagnosis.

Imaging obtained early may show less than a study performed weeks later, so the sequence of scans matters.

Why was my CT scan normal?

CT is good at detecting bleeding and fractures but poor at detecting microscopic fiber damage. Specialized MRI sequences reveal considerably more.

Can this occur without loss of consciousness?

Milder forms can occur without prolonged unconsciousness, though the severe form typically involves it.

Is recovery possible?

Some recovery occurs, particularly in the first year, though significant permanent deficits are common in more severe cases.
